Who is the High Impact Change Model: Optimal Handed Care for?

The High Impact Change Model: Optimal Handed Care is intended for use by ‘Health and Care Systems’, a collective term used to describe local partners inclusive of NHS bodies.

It is intended for use by ‘Health and Care Systems’, a collective term used to describe local partners inclusive of NHS bodies (including Integrated Care Boards, NHS trusts and NHS foundation trusts); local authorities; social care providers; voluntary, community and social enterprise (VCFSE) organisations; and other partners such as domiciliary care providers and care homes who have a critical role in enabling people and informal carers to maximise their independence and dignity. In addition, engagement with faith organisations can provide insights into cultural norms that may prevent this approach being fully implemented. 

The model highlights the changes in areas across a person’s journey that will have the highest impact on:

  • the effectiveness, quality and outcomes for people with Optimal Handed Care needs.
  • efficiencies and savings that can be made through a more proactive focus on mobility, function and maintaining independence.

It is applicable to all population cohorts and should be implemented across all pathways where a person’s comprehensive assessment prescribes Optimal Handed Care.
